Eligibility

Before turning out for a sport, students must complete three permission forms and submit them to the Athletic office. The first two forms below must be renewed and submitted each year a student intends to participate on any Holy Names Academy athletic team:

1. Holy Names Academy Athletic Participation Agreement & Health Information: Valid for 13 months from date signed; a new form must be on file with the Athletic Department each year the student intends to participate on any HNA athletic team. This form requires a physician's signature.

2. Concussion and Head Injury Information Sheet: Must be filed with the Athletic Office each year a student intends to participate on any Holy Names Academy athletic team. Requires signature of the student AND of a parent/guardian.

3. WIAA Pre-participation History and Physical Examination: For incoming 9th graders, transfer students, or current HNA students who are participating in HNA athletics for the first time. This form requires a physician’s signature, and need only be filed with the Athletic Department once during a student’s time at Holy Names Academy.

The athletic program adheres to HNA's policies forbidding the use, possession or sale of illegal drugs or alcohol at any school-sponsored events.

Smoking is prohibited at all school-sponsored events and is not allowed within two blocks of the school.

Transportation to all athletic practices and events is by school minibuses unless the Athletic Director receives written approval from parents/guardians for other arrangements.
